Are you immune from switch places in the person playing is still on the entrance square?
Yes, someone cannot be switched when not in the house. If you are not in the house, you can not play the Switch Places card.

Can players move from the outside entrance, to the inside of the building and back out again?
No. Once inside the house you can't leave through the front entrance.

Can you gain access to the house using a trap door card?
You can gain entrance to the house with the Trap door card. You can fall through the trap door on the porch or find a secret passage though.

Can you use a two space move to exit at a range of 1 square?
Yes. You just have to be the first one out of the house. It doesn't matter how far.

If a Secret Corridor is drawn and no single walls are present to sneak through, do players move along the corridors as if they were moving ordinarily, or do they not move at all?
If there are no walls to go through with the secret passage, no action is taken.

What happens if players start The Second Story on an Icon Tile? Does it constantly revolve? Who gets the Disk? What if a Ghost is also present there? Wouldn't it be wiser to make them start on a blank tile?
Tiles only revolve when first landed upon, so it would not rotate at the beginning of the game. Same goes with the disk, you only get a disk for tiles that you land on. So, the disk is not even looked at until someone moves off the tile and then back on again. If it is a ghost that is flipped when other players are on it, they don't see it until they move off the tile and then back on again. You can start on a blank tile if you don't want to deal with these sorts of things.

When the 'Switch Places' card causes one or both of the pawns to wind up on an icon tile, does the icon trigger to rotate the room?
No. The icons are not triggered by 'Switch Places' because there really isn't any 'movement' to cause a piece to 'land' on a square. 'Landing' on the square is what triggers the icon.
